Biblical Counseling Questionnaire

If you are interested in meeting with us to discuss what you are going through, please print and fill out this Personal Data Inventory sheet. This helps us to have a full understanding of who you are, where you are coming from and what you are looking to accomplish.

After filling this out, either email it to Pastor Bob at bobscott@cedarcreekmi.org or stop by and drop it off. We will take a few days to look it over and give you a call for either further information or to schedule our first appointment together.

Our courtesy for women is that men are not allowed to counsel women alone, so if you are a woman in need, we will have a female counselor available as well.
